Our Services Include:

Hospice Care

Available 24/7 (including nights, holidays & weekends), we provide the four levels of hospice care that help address the needs of your loved one during their end-of-life journey: Routine Home Care, Continuous Home Care, General Inpatient Care, & Respite Care.

Symptom & Pain Management

Our Medical Director and your entire care team continuously monitor changes in your loved one’s health to ensure pain and disease symptoms are managed and appropriate medications are coordinated when needed to ensure your loved one’s pain is managed and is as comfortable as possible.

Individualized Care Plans

Everyone is different and requires different things… and we’re more than happy to accommodate where we can. Being able to create a personalized care plan for each patient allows us to deliver on our mission of providing high quality and compassionate end-of-life care.

Spiritual & Emotional Support

Regardless of one’s religious denomination or spiritual beliefs, we are happy to provide emotional support and resources to you and your loved one based on their personal beliefs, values, traditions, and preferences.

Bereavement & Grief Support

Our bereavement and grief support services aim to help families and their close community process their grief and find healing over time. Through support groups, counseling, educational resources, assistance with services or remembrance if requested and so much more; we believe that hospice care extends to the entire family and we’re here to help when needed most.

Volunteer Visits

Volunteers do so much as a part of your hospice care team! Volunteers can be scheduled to provide companionship & conversation, play games, reminisce, perform light duties and more that allow you to have breaks and spend quality time with your family and loved one.

Integrative Therapies

Non-medical therapies can be extremely beneficial for relieving physical, emotional and mental symptoms. Available to all patients receiving care (unless physician order states otherwise), our integrative therapies include musical support, pet visits, and massage therapy that can assist with symptoms management and improve one’s overall quality of life.

Your Care Team

The Superior Hospice care team plays an important role in ensuring your loved one receives the care they deserve during their life transition. Your hospice care team includes, Physicians, Nurses, Therapists, Pharmacists, Social Workers, Spiritual Care Coordinators, Home Health Aides & volunteers that work together to provide a holistic person and family-centered approach to care. The entire care team is available to you and your loved one to coordinate care, provide education and support, and address your loved ones emotional, spiritual, and social needs alongside their physical care. We’re just a phone call away and available 24/7!

Spiritual Care Coordinator

The Hospice Spiritual Care Coordinator provides spiritual and/or religious services to the patient, their family, friends & community. They can assist with counseling for patients and grieving families, spiritual support to caregivers, crisis intervention and so much more to provide comfort and solace in difficult situations. The hospice spiritual care coordinator may also act as a liaison to help develop a care plan or address advance healthcare directives.

Pharmacists

The Hospice Pharmacist plays an important role as a member of the hospice care team. They address and assist in managing complex conditions and their symptoms as well as monitoring medications and potential drug interactions. Through collaboration with other members of the care team, pharmacists can help ensure each patient receives the end-of-life experience they deserve.

Nurses

The Hospice Nurse works with every member of the hospice care team to provide a personalized care plan to provide dignity and respect as the patient nears end-of-life. They track, identify and manage symptoms, educate patients & families, provide mental, physical and emotional support, obtain needed supplies and medical equipment, and collaborate with other specialists to ensure quality-of-life.

Volunteers

The Hospice Volunteers provide many important services to the patient, their family and close community. They provide direct support to the patient and their family, assist with administrative services, offer companionship through conversation, music, games, stories and more. Most importantly, hospice volunteers provide relief to family caregivers so they can spend valuable time with the patient and their loved ones.

Integrative Therapies

Hospice Integrative Therapies play a vital role in providing non-medical cares to assist with relieving physical, emotional and mental pain and symptoms. These therapies include: massage therapy, musical support, pet visits and more to ensure the patient is comfortable and pain free as the patient nears end-of-life.

Social Workers

The Hospice Social Worker is an essential member of the care team. They advocate for the patient and their support system to ensure the right resources are available. The hospice social worker can provide care planning and assistance, emotional support to both the patient and their support network, as well as lead support groups and provide education and training on topics like coping with grief, fatigue and end-of-life decision making.

Therapists

Physical, Occupational and Speech therapists are important members of the care team that assist with improving the quality of life for patients and supporting families. They can assist with managing symptoms like weakness or difficulty breathing, modifying the home to help adapt to physical changes and even addressing communication and swallowing difficulties. While their goals are not focused on rehabilitation or long-term recovery, they do provide valuable cares and activities that assist with maintaining dignity.

Hospice Aides

The Hospice Aides are an important member of the care team that provide assistance with activities of daily living, personal cares, meals and dietary needs, transferring & repositioning and even keeping their space clean and safe. They serve as a friendly face that provides gentle and personalized cares to the patient and their family to ensure comfort and support.

Physicians – Hospice & Attending

The Physician plays a vital role in the patients care. Both the Hospice Physician and the Primary Care Physician are skilled in managing medical symptoms and coordinating the needed cares and leading the entire hospice team in providing care. In addition to managing the patients overall care, physicians provide support the hospice team and share education and resources with patients and their families.

Family Friends & Community

Immediate Family, Friends & Close Community are also an important role within the patient’s hospice care team! They provide emotional support, help with caregiving, and advocate for the patient’s needs. The partnership between the patient’s family and the hospice care team enhances the quality of care, ensures the patient’s dignity, and creates an environment where both the patient and their loved ones feel supported.

Patient

The patient is the central figure and most important member of the hospice care team because their needs, preferences, and experiences shape our entire approach to their care. They define their end-of-life goals, communicate their needs and concerns and truly guide the entire care plan. Their voice is essential in creating an environment that prioritizes comfort, dignity, and fulfillment at life’s end.
