Anyone can make a referral to hospice, even without a physician’s note! Next, you’ll receive a free care consultation where our team will learn about your loved one’s condition and care goals, either in person, by phone, or virtually. Then, we’ll schedule a hospice assessment to better understand your loved one’s needs, explain services, and answer questions. Finally, if eligible, we’ll proceed with admission to care, ensuring a smooth, compassionate transition into hospice support.
