Our Services Include:
Hospice Care
Available 24/7 (including nights, holidays & weekends), we provide the four levels of hospice care that help address the needs of your loved one during their end-of-life journey: Routine Home Care, Continuous Home Care, General Inpatient Care, & Respite Care.
Symptom & Pain Management
Our Medical Director and your entire care team continuously monitor changes in your loved one’s health to ensure pain and disease symptoms are managed and appropriate medications are coordinated when needed to ensure your loved one’s pain is managed and is as comfortable as possible.
Individualized Care Plans
Everyone is different and requires different things… and we’re more than happy to accommodate where we can. Being able to create a personalized care plan for each patient allows us to deliver on our mission of providing high quality and compassionate end-of-life care.
Spiritual & Emotional Support
Regardless of one’s religious denomination or spiritual beliefs, we are happy to provide emotional support and resources to you and your loved one based on their personal beliefs, values, traditions, and preferences.
Bereavement & Grief Support
Our bereavement and grief support services aim to help families and their close community process their grief and find healing over time. Through support groups, counseling, educational resources, assistance with services or remembrance if requested and so much more; we believe that hospice care extends to the entire family and we’re here to help when needed most.
Volunteer Visits
Volunteers do so much as a part of your hospice care team! Volunteers can be scheduled to provide companionship & conversation, play games, reminisce, perform light duties and more that allow you to have breaks and spend quality time with your family and loved one.
Integrative Therapies
Non-medical therapies can be extremely beneficial for relieving physical, emotional and mental symptoms. Available to all patients receiving care (unless physician order states otherwise), our integrative therapies include musical support, pet visits, and massage therapy that can assist with symptoms management and improve one’s overall quality of life.
