We’d love to hear from you — simply give us a call at 763-277-8777, or fill out our contact form so we can connect with you. You can also request a referral for Superior Hospice from your physician or another community member.

In our complimentary in-person or phone consultation, we’ll learn more about your needs, answer any questions you may have, and help you determine if it’s time for hospice. Following our initialinformational visit our Director of Hospice will meet with you for an in-depth assessment of hospice eligibility and begin discussing your customized care plan.
Once the formal admission process is complete, we’ll begin your hospice journey right away. Throughout your hospice journey, you’ll receive exceptional care catered to your needs, have access to our extensive education and resources, and be looked after by a care team that’s devoted 24/7 to your comfort.
