WHAT IS CONTINUOUS HOME CARE?

Continuous Home Care provides up to 24 hours of nursing and aide support in your home during a medical or emotional crisis. The goal is to stabilize symptoms and prevent the need for hospitalization — so your loved one can remain in a place of comfort and connection.
